Great Lakes Practice Transformation Network

The Great Lakes Practice Transformation Network is a region-wide collaboration led by the Indiana University School of Medicine with partners in Illinois, Ohio, Indiana, Kentucky and Michigan. With funding from the U.S. Department of Health and Human Services, Centers for Medicare and Medicaid Services, the goal of the program is to guide 15,000+ clinicians through the patient-centric practice transformation process to improve practice approach and better align their practice for value-based payment system reform.

Upper Peninsula Health Care Solutions, Inc. works with Altarum Institute, the Michigan and Ohio program lead, to serve the Upper Peninsula.

In her role as Quality Improvement Advisor, Marlene Beaudry guides clinic staff through the transformation process: assisting in process and workflow improvement; implementing new clinic procedures, and providing education to ensure participating providers can earn Continuing Medical Education and Maintenance of Certification credits. Kristopher DeLoose, Data / Application Specialist, assists clinics in the HIT realm, helping them to retrieve and understand their data and navigating Quality Payment Programs.
